Appointments:

Call us at 770-476-4144 for an appointment or with any questions. Our hours are 9:00am – 5:00pm Monday through Thursday. We are closed for lunch 1:00pm-2:00pm. We will work with you to set the most convenient appointment time available.

Please call 24 hours ahead of your scheduled appointment time if you must cancel or reschedule your appointment.

Payment is expected when services are rendered. See the financial policy section of our website, or ask our office personnel about payment options.

First time patients:

To save you time and inconvenience first time patients are invited to download the following forms and bring them filled out and signed to your first appointment.
